List an email address where you can be contacted during the summer camp. If you do not have one, list a parent or guardian's email.
Give the full name of your parent or guardian.
Give an email for a parent or a guardian who can be reached during the summer camp.

List any days of the summer camp that you would not be able to attend (if any).
